Constipation on Ozempic: A Dietitian's Fix-It Plan

Constipation on Ozempic happens because the medication slows your digestive system while you simultaneously eat and drink much less, which removes the bulk and fluid your bowel needs to work. The fix is deliberate: a daily fibre target of 25 to 30 g, around 2 litres of fluid, regular movement, and kiwifruit or prunes doing the heavy lifting. This is the plan that solves it for most of my clients.

Why it happens

Three things stack on top of each other. Semaglutide (Ozempic/Wegovy) slows gut motility directly. Your food volume drops, so there is less bulk to move. And many women drink less without noticing, because the thirst that used to come with meals fades when meals shrink. Less material, less water, slower transit: constipation is the predictable result, not a mystery.

That also means it responds well to nutrition, because you can put back what the medication and small appetite took out.

The fix-it plan

1. Hit a fibre number, not a vibe. Aim for 25 to 30 g daily. On a small appetite that requires density: legumes (half a cup of chickpeas is 7 g), high-fibre cereals like All-Bran or Weet-Bix, chia seeds (2 tablespoons is 7 g), pears and kiwifruit with skin on, grainy bread over white. Track it for three days; most women on GLP-1 medications are under 15 g without realising.

2. Two kiwifruit a day. Green kiwifruit has clinical trial support for constipation, works gently, and is small enough for a suppressed appetite. Prunes (4 to 6 daily) are the traditional alternative with similar evidence.

3. Fluid is half the battle. Fibre without water makes things worse. Sip toward 2 litres daily, more in an Australian summer. Warm drinks in the morning help some people get things moving.

4. Move daily. A 20 to 30 minute walk stimulates the bowel. It does not need to be exercise-shaped; it needs to be regular.

5. Respect the urge. Slowed transit means fewer signals. When one arrives, go. Deferring it repeatedly trains the bowel in the wrong direction.

6. Consider a fibre supplement as a bridge. Psyllium (Metamucil and equivalents) is a reasonable stopgap while food fibre climbs, start low with plenty of water. What a high-fibre, small-volume day looks like

  • Breakfast: Weet-Bix with milk, chia and a kiwifruit (10 g fibre)
  • Lunch: grainy toast with cottage cheese and tomato, a pear (8 g)
  • Snack: high-protein yoghurt with 2 prunes (2 g)
  • Dinner: palm of chicken, half a cup of lentils through roast vegetables (9 g)

That is nearly 30 g of fibre inside a modest day of food, and it still hits the protein-first structure from what to eat on Ozempic. The two goals do not compete; they share a plate.

When it is more than a nutrition problem

See your doctor if you have no bowel motion for several days despite the plan above, severe bloating or abdominal pain, blood in your stool, or alternating constipation and vomiting. Your prescriber may adjust your management, and constipation this stubborn should not be self-treated indefinitely with laxatives.
